A care-coordination platform for US hospitals — tracks cardiac patients after discharge and alerts care teams to readmissions from live HL7 feeds.
Zilius helps hospital care teams follow cardiac patients — after bypass surgery or a coronary intervention — once they leave the hospital, so readmissions are caught and acted on early.
We worked on the platform end to end: data views for patients, physicians, facilities and admission events with search, filters and CSV export; monthly readmission alerts; ingestion of HL7 ADT messages from a regional health information exchange; and data uploads and reporting. Each hospital runs its own isolated deployment on AWS, with Cognito sign-in and a GraphQL API over MongoDB.
